Decoding Achilles pain: What it is – and what it’s not


Sometimes, your ambition outpaces your body. When the Achilles tendon goes on strike, that is a clear signal from your body that the balance between load and recovery has fractured. Ignoring it is not an option. You need to understand the mechanics of the injury. And then you need to take action.

Achillodynia: Why does Achilles pain occur?


The medical term achillodynia describes what is usually an overwhelmed tendon. Think of it as a robust steel cable: extremely resilient, but not indestructible, and susceptible to wear and tear.

Frequently, it presents as a tendinopathy, better known as Achilles tendon inflammation or irritation. The core issue here isn't the acute inflammation but the compromised load tolerance.

The Achilles tendon is the strong tendon at the back of the ankle. It connects the calf muscles to the heel bone and transfers force when walking, running, jumping and climbing stairs.

Where is the Achilles tendon? Typical Achilles tendon pain is usually felt directly along the tendon, most commonly 2–6 cm above the heel bone. Pain directly under the heel or on the side of the ankle is more likely to be caused by a different condition.

From recovery to running again


Achilles pain can interrupt training, but the right recovery routine can support the way back. These steps show how treatment, support and a gradual return to movement can work together.

Typical symptoms related to the Achilles tendon


Start-up Pain

The first few metres after rest feel stiff and painful. After a few minutes of moderate movement, the discomfort subsides – only to return twice as strong after the load.


Exertional Pain

Typical symptoms include a stabbing pain at the back of the heel and a sensation of pressure 2-6 cm above the heel bone. Even climbing stairs can trigger it.


Conversely: If the primary pain is not above the heel, along the tendon, and the characteristic start-up pain is absent, the cause might lie elsewhere. When in doubt, always let a professional diagnose it.

Achilles bursitis or inflammation of the Achilles tendon bursa: If the pain is located directly at the heel bone and is accompanied by pressure from shoes, redness or swelling, the bursa (subachilleal bursa) may also be irritated. This is a different pain mechanism from classic Achilles tendon irritation and should be assessed by a medical professional if symptoms are persistent, severe or unclear.

What to do when the Achilles tendon hurts?


A first-aid plan is helpful for managing acute symptoms. The RICE rule has proven effective for alleviating Achilles tendon discomfort:

Rest

Give the tissue time to rest. Reduce athletic load to a minimum. If symptoms persist after a week, seek medical advice.

Ice

Cold relieves pain and swelling. Use a cool pack or cold towel for 10-15 minutes, never directly on the skin.

Compression

Apply compression. Gentle pressure can help minimise swelling and stabilise the tissue.

Elevation

Keep the affected foot above heart level to promote fluid drainage. Ideally, elevate it overnight while sleeping.
